Please read prior to booking your program.
Our programs are group medical visits and you will be sharing this experience with other participants. The programs run either 4, 6 or 8 consecutive weeks depending on the program and their sustainability depends on you attending all session in your program. Please ensure that the program you register for is a good fit for you and that you have the time and the commitment to participate in this program.
Program Fees:
Our programs are a combination of MSP covered group visits and administration fee to the patient. There is an upfront cost of $85.00 that is required at the time you register for our programs. From the $85.00 charged, $45.00 is a registration and administration fee for all programs that is non-refundable and important for the sustainability of this service. The $40.00 no-show fee is collected and is returned upon completion of the program with full attendance to all sessions or up to 1 missed session. If you miss 2 or more sessions for any reason, the no-show fee will NOT be returned.
Refunds:
All refunds will be processed at the end of the month of which the program ends.
Cancellation Policy:
There is a full refund of all fees for cancellations received up to 14 days prior to program start date. No refunds when less than 14 days. Firm policy.
Program Procedures:
Your enrolment in the group means you have agreed to attend all sessions and complete the assigned weekly home practices. At course completion, we will ask you to fill out an evaluation form and offer suggestions for improvements. Please be aware that your facilitator(s) will track your experience in the program by completing weekly notes, which is required as part of the MSP funding arrangement. At the completion of the program, your family physician will receive a brief summary note from your facilitators.
Potential Risks of Participating in the Program:
Some of the material discussed during the programs might upset you. Please let your facilitator know. Your family physician remains the Most Responsible Physician throughout the program, so make an appointment with your doctor if you feel you need counselling support.
Potential Benefits of the Program:
This program and its related materials were designed to benefit you. The materials are intended to help you reflect and apply the teachings to your own situation. The workbook/handouts can also serve as a reference resource after you complete the program.
Confidentiality:
Your confidentiality will be respected. Only your family physician, your facilitator(s) and our administration assistants will have records of your name. We have taken all measures to ensure your information is kept secure during its transmission, storage, reporting and destruction. All participates data and records are kept in our secure EMR (Electronic Medical Records). In terms of confidentially for the group medical visits, please be aware that in this setting you may discuss some aspects of your health, and hear about other people’s personal health information. As a matter of trust, it is important not to talk about other people’s personal medical issues outside of this circle setting. Of course, you are welcome to discuss things you have found helpful with friends and family, providing nothing that you say identifies others.
Participant Consent:
Taking part in a group is voluntary. If you do decide to take part, you may choose to discontinue your participation at any time without giving a reason and without any negative impact on your access to other services in the community. Completing evaluation forms is also voluntary.
