Responsibilities:

Prepare and submit clinical and progress summaries based on identified and attained goals
Individualize treatment plans based on patients' needs, progression, comfort and personal goals
Preferential responsibilities:
Registered Physical Therapist or eligible to register with the College of Physical Therapists of British Columbia
Minimum 2+ years of experience is preferred
IMS/dry needling and/or Acupuncture is an asset (but not essential)
Have great interpersonal and communication skills; collaborative healthcare experience is an asset!

About the Job:
We are seeing an energetic and experienced LPN to support our dynamic team in providing primary care and women’s health to patients. The LPN will work in a fast paced, high volume practice environment that facilitates patient access to appropriate and timely care.
Work within an integrated healthcare philosophy to improve communication and coordination of patient care.
Roles and Responsibilities:
Nursing:
Triage patients (including take vitals and brief histories)
Prepare patients for examination by performing duties such as escorting patients to the examining room, weighing patient
Prepare examining rooms for patient consultations and examinations by performing duties such as checking and laying out clean instruments, stocking rooms with required supplies, cleaning and making up room for next patient and collecting used instruments and processing for sterilization
Coordinates patient care through the exam rooms using universal infection control precautions
Manages patient advice calls to discuss test and provides patient instruction only as advised by physician
Assist medical staff with physical examinations by performing duties as such as handing and receiving instruments and supplies, receiving and preparing specimens, attaching completed requisitions and placing in receptacles and/or delivering to the laboratory
Perform a variety of nursing procedures such as performing duties such as applying and changing sterile or non-sterile dressings
Record observations
Administer injections and immunizations: provide basic related patient education
Provide cervical screening (Pap tests)
Document encounter data and patient history in EMR
Consult with or refer patients to fellow members of the interdisciplinary health team as appropriate
Provide support counseling as needed
Monitors, tracks and records all preventative care and screening to including mammograms, paps, colon cancer screening and annual mental health assessments
Competent or interest in training in clinical aspects of the office including application of dressing, staple and suture removal, administration of immunizations and injections, preparing for and assisting in other minor procedures (paps, ear syringing, skin biopsy)
Competent in sterilization procedures for office equipment
Administrative Duties:
Pleasantly greets all patients both during scheduling, registration process and when answering all phone calls. Respectfully redirects calls as needed. Triages and prioritizes the relay of messages promptly
Schedules patient appointments daily and as required to optimize flow and minimize downtime
Ensures the electric medical record (EMR) patient information such as address and pharmacy of choice, next of kin is accurate
Ensures all patient forms are scanned and tracked
Perform a variety of clerical duties such as organizing appointment lists and reports for clinic and ensuring charts are returned to order processing after examinations, answering telephone, filing and typing utilizing variety of software packages
Responsible to pick up, distribute and send mail, shred material daily as required
Ensures that all incoming documents (electronic or by Fax) are scanned appropriately to EMR
Documents all communication with patients in the EMR and patient chart accurately and concisely at the time of encounter
Facilitates and coordinates all meetings for staff, physicians
Arranges for referrals/consults as directed by physicians, keeping referral bank current. Calls patient consult/referral date is received
Orders administrative/medical supplies as needed. Develops and maintains spreadsheet of inventory. Stocks exam rooms and nursing statin daily with medical supplies
Collaboratively works with health and safety to ensure that all medical equipment, emergency box, such as oxygen tanks, liquid nitrogen, are checked a minimum weekly and keeps a record
Demonstrates flexibility to accept changes to areas of responsibility as required
Qualifications and Skills:

About You:
We are seeking a candidate with the following qualifications:
Graduation from an accredited Licensed Practical Nursing Program
Current CPR and First Aid certification
Hold professional membership and professional liability protection insurance
A clear police vulnerable check within the last 3 months
Requirements:
Maintains confidentiality at all times
Work in collaboration with Lifestyle Medicine, Physiotherapy
Professional in attire required
Current registration and in good standing with the College of Nurses of BC as an LPN
Current full practicing licensure with British Columbia College of Nursing Professionals (BCCNP)
Demonstrate proficiency in all applicable computer programs (ie: MedAccess system an asset)
Ability to work remotely during pandemic if necessary
Assume other duties as appropriate
Participates in data collection activities as required
Ability to remain sitting for considerable periods of time, ability to communicate extensively over the telephone
Asset:
One to three years of experience in a clinic and or family practice office / primary care setting preferred
Knowledge and background in women’s health and psychiatric wellness
Solid understanding of social determinants of health and health equity
Experience with well-woman care (including pap tests) an asset
Proficiency in MedAccess EMR
Psychiatric nursing experience or interest
Recent community experience and adequate knowledge of community resources
Position:
3 – 5 days/week
Hours: 24 – 35 hours per week - 6 month contract with goal of developing into a full-time position.
Salary: based on primary care nursing standards and individuals experience.
The hours will generally be worked as follows: 3 - 5 days, 8:00 am to 4:00 pm, subject to change as needed to accommodate patient needs.
There might be a requirement to work 1-2 Saturdays a month (8:00 am to 12:00 pm) in lieu of another workday.
